Patient Directions for Telemedicine

  • Download and complete the Consent for Telemedicine Services.
  • You will need to email the completed consent form to telemed@yardleyderm.com (you may take a photo or download and return). See photo tips below.
  • Put the Physician’s Name you wish to schedule with into the subject line of the email.
  • Once the consent is received, a receptionist will call you back to schedule your telemedicine appointment. They will review demographic and insurance information as well as collect your co-pay (if applicable).
  • We will send you a confirmation email with the link to connect you with your doctor (you will need to allow access to your camera on your device).
  • Patients who do not have computer or FaceTime access may conduct their appointment over cell or landline.

Tips for taking photos so they’re helpful in making a diagnosis

  1. Make sure you’re in a well-lit room, with lighting behind the camera (i.e., stand in front of a light and hold your camera between you and the light).
  2. If you are using your phone, focus on the area you are trying to photograph. Then try holding down on this area to sharpen the focus (not all phones have this feature).
  3. Take photos of each spot or area from a few different angles.
  4. Get some assistance from someone else if needed.
